NHS patients have a legal right to pick their first outpatient appointment. Some GPs don't understand how it works. Siobhan Rourke found this out when she attempted to get an ADHD assessment through Right To Choose.

Right to Choose is the legal right that NHS England gives patients to select their own team of doctors to provide non-urgent treatment led by consultants. This includes psychiatry. This does not apply to services that are ordered by local authorities.

The findings from this research revealed that GPs relied on their pre-existing knowledge and relationships to inform their referral decisions more than the particular needs of patients. The recorded meetings demonstrated this. CMHT leaders and Psychiatrists also spoke out about their views on the competence of the individual GPs, and this was evident in all of the recorded meetings.

The current pandemic has resulted in an enormous increase in number of people being referred to community mental health teams for support. This has resulted in a significant increase in need for psychiatry assessments as well as an increased workload for staff. The rapid onset of the pandemic meant that it was difficult for GPs and other healthcare professionals to handle the volume of referrals. This led to a lot of uninformed referrals.
In response, we conducted a qualitative study that was embedded within an uncontrolled trial that was randomised to examine the effectiveness of a range of strategies that aim to improve the quality of referrals for mental health. Interviews with GPs and community mental health team consultants (CMHT) were conducted, as was the recording of meetings to aid in the distribution of referrals. The findings of the study showed that the use of a bespoke referral route was a successful method of reducing the workload of GPs as well as CMHTs and also provide patients with the options they need to be treated effectively.
